Yes, you do have to quarantine upon returning back to North Korea.
Travelers are subject to 14 days of mandatory self-isolation at home, declared location or location designated by authorities.
You have to self-isolate for the full duration ( 14 days) and it is not possible to end the quarantine early by taking a repeated test.
